
We all want to take care of our loved ones in their times of need—being there to reassure, support, and safeguard their health and dignity.
Unfortunately, caregiving is usually a full-time job. Well-meaning family members run themselves ragged trying to keep up with daily tasks, doctor visits, health monitoring, meal prep, etc. Issues of distance (living in another city or state), time (an already full schedule), and knowledge (who to call for help and when?) can weigh down an already tired spirit, turning what was once quality time into yet another have-to.
The answer is care management.
What is care management?
Care management is an array of services and activities enabling people with chronic or complex conditions to manage their health and life properly. A care management team may include registered nurses, social workers, art therapists, occupational therapists, qualified care providers, family members, and more. It includes:
A good management care program will establish realistic short- and long-term goals, coordinating with available community services to make sure the treatment being provided is appropriate, dependable, and of the highest quality.
Proper care management looks after everyone involved. This includes loved ones, caregivers, responsible parties, adminisrators, and support staff.
Arizona Fiduciary Services provides a wide range of care management services for fiduciary clients in need of in-home assistance and/or monitoring in care facilities inclu.ding:
● Home care needs assessment.
● Facilities assessment and referrals
In home or institutional care evaluation and monitoring.
● Physician and nursing staff coordination and advocacy.
● Determination and/or coordination of placement for all levels of care.
● Supplemental, social, medical and companion services arrangements, including adult day care and other personal care services (through accredited agencies).
● Agent under Health Care Directives (available only to those persons for whom we are acting as trustees or successor trustees).
We will assess, plan, and oversee your loved one’s treatment, providing crucial support to both of you. Our support services also include money management, assessments and evaluations. We handle care management for loved ones in Tucson, Marana, Oro Valley, and Green Valley as well as out of state.
IMPORTANT: We do not provide direct care—rather, our role is to manage the services received. We have the experience and know-how to make sure your loved one lives safely and well.
